Attention Incoming Student Parent/Guardians - 6/10/2010
ATTENTION PARENT/GUARDIANS OF NEW STUDENTS WHO ARE COMING FROM A SCHOOL OUTSIDE OF GRAND RAPIDS PUBLIC SCHOOL BOUNDARIES (Example – Holy Trinity, Assumption, Walker Charter):
Michigan Law requires that students have a complete record of immunizations to enroll for school each year. Beginning in the fall of 2010 new immunization requirements will go into effect.
· Students at least 11 years of age or older who have not had a tetanus/diphtheria containing vaccine (DTap, DT, Td, Tdap) within the last five years are now required to have one dose of tetanus/diphtheria/acellular pertussis vaccine (Tdap). They must have the Tdap instead of Td for their booster dose.
· Students at least 11 years of age or older are also required to have one dose of the meningitis vaccine (MCV4 or MPSV4).
· Students at least 4 years of age and older who are enrolled in school are required to have two doses of the Varicella vaccine or history of chicken pox disease.
All Students who are entering kindergarten, 6th grade or as a new entrant to the school district this fall are required to present a written record verifying their immunizations to their schools in order to attend classes. Students who are not up to date on their immunizations are subject to exclusion from school.
It is very important to schedule a time for your child to have these immunizations as soon as they are eligible to receive them. Please contact your primary care physician’s office to schedule an appointment.
The Kent County Health Department is a Vaccines For Children (VFC) provider. Children up through age 18 without insurance, have insurance that does not cover immunizations, or who have Medicaid receive vaccines at no charge. An administration fee for each vaccine still applies and will be charged according to a sliding fee scale.
If you will be taking your child to the Kent County Health Department, please note that immunizations now require a scheduled appointment. Please call 616-632-7200 to schedule a date and time at the most convenient of the six clinics. Walk-ins are no longer accepted.
If you have questions, please contact your health care provider or your school (233-5900). Kent County Health Department information can be found at www.accesskent.com/immunizations.
